INSTRUCTIONS
 
  1. Right-click the link (ANS-form.doc) or hold down the mousebutton for a few seconds if using Mac,
    Save Target/Link As...  Do not open the file within the browser!
  2. Browse to the folder you want to save the file in, Save.
  3. When done downloading, open the file you just saved and fill out the form.
  4. Save the file as "LastnameFirstname.doc".
  5. Return the form to ANS as an email attachment.
